Job purpose:

Promote choice, provide appropriate support, and coordinate activities to foster individual wellbeing, growth, and development and ensure that all services, programs and guidelines outlined in individuals’ ISPs are implemented.

Duties and Responsibilities:

  • Case management for individuals that attend MDAC, including IP/SEMI, case record are in accordance with TKC/DDS guidelines
  • Works closely with the program manager to ensure program policies/procedures are followed
  • Works closely with the program manager/art specialist to facilitate artwork, assist with art shows/events
  • Ensure that all Kennedy CollectiveSM and DDS regulations and policies are adhered too. Participate in all Quality Service Reviews.
  • Responsible along with the Manager on training and supervision of all assigned staff.
  • Will assist the manager in monitoring and approve staff's timecards and PTO request in ADP. 
  • Responsible for the supervision and coordination of monthly calendars, and recreational activities
  • Manage the petty cash and purchases for the programs, keeping within the guidelines and budget
  • Provide professional and timely communication with co-workers, supervisor, families and guardians and other team members.
  • Record and maintain individuals record in Therap as assigned, Data entry, case notes, updated contacts, and information. 
  • Audit staff's entries into Therap for compliance.
  • Work directly with the program nurse to ensure all medical needs of the individual are met, doctors’ orders and protocols are in place.
  • Administer mediations in accordance with DDS regulations.  Ensuring all medications are available and orders followed. 
  • Advocate for the individuals whenever necessary as well teach individuals to advocate for themselves. 
  • Ensure the protection of all individuals rights and safety by following HIPAA regulations, ensuring privacy, and reporting any type of abuse or neglect. 
  • Intervene in emergencies or behavioral situations as needed.
  • Assist the manager in ensuring that GER's, DDS 255 and critical incident reports are completed in a timely manner.  Complete all resolutions to the GERS in a timely manner.
  • Maintain a comfortable home or program space, ensuring a clean and safe environment.
  • You are required to be in possession of a car on site during your entire shift to transport yourself and/or individuals served to community sites in all weather conditions and throughout all neighborhoods.
  • Complete annual assessments as assigned. 
  • Accompany individuals on community outings.
  • Ensure all wellness checks are completed and documented as required.
  • To be on call for programs as assigned. 
  • Audit Easy clocking system entries for accuracy 
  • Other related duties and responsibilities as assigned. 

Job Requirements

Qualifications:

  • High School Diploma.
  • One year of experience working in the human services field 
  • Valid Driver’s License 
  • Must maintain a satisfactory driving record.
  • Must maintain PMT and CPR certifications.
  • Knowledge and skills in Microsoft Office and the internet.
  • Work a flexible schedule that may include early mornings, evenings, weekends and/or holidays.
  • Compassion and patience for those you work with
  • The ability to be professional while representing the individuals and agency
  • Communicate clearly in oral and written form on a constant basis.
  • Work independently; utilize good judgment and possess a strong sense of initiative.
  • Do math calculations, use a calculator, tape measurement and a scale.

Physical Requirements:

  • Provide hands on personal care which may include toileting, feeding, cleaning and all other activities of daily living skills.
  • Provide physical interventions which may include lifting, mobility, first aid, and safety evacuations.
  • Have physical endurance to use physical crisis intervention techniques which may include restraining individuals.
