Job Description

Key Responsibilities: 
  • Provide care to individuals according to their level of supervision, level of need, individual plan, and behavior support plan (if applicable). 
  • Maintain confidentiality regarding individuals and sensitive information, including HIPPA information. 
  • Maintain a secure, positive environment that nurtures individual's needs, self-expression, and goals. A positive environment includes; healthy relationships with the individuals, their loved ones/advocates, coworkers, supervisors, consultants, and any member of the public while representing the company. It is important to provide a good example for navigating relationships within our community. 
  • Daily support and engagement through activity planning, goal achievement, social interactions, skill-building, and recreational activities that encourage creativity, teamwork, and self-exploration. 
  • Assist individuals with Activities of Daily Living (ADL's) such as bathing, dressing, grooming, and meal preparation as needed. 
  • Ensure residents' rights are respected and upheld at all times. 
  • Provide individual observation to monitor health and behavioral changes, reporting any concerns to the appropriate personnel. *We have a 24/7 on call nursing line. 
  • Cater to the unique needs of each individual by providing hands-on assistance in daily tasks, communication support, and emotional encouragement. 
  • Collaborate with nursing staff and other professionals to coordinate care effectively. 
  • Maintain accurate documentation of services provided (through paper systems and Therap as required) and participate in team meetings to discuss progress. 
  • All individuals homes are upkept for cleanliness and safety. 
  • Utilize company vans to transport individuals to and from community outings/events or medical appointments as needed. While obeying all traffic laws and abiding by all safety precautions. 

Job Requirements

General Requirements:
  • A valid CT or RI driver's license. 
    • If you have a license from a different state, you will have 90 days to switch it to CT as required by CT state law (not including RI). 
  • A clean background check to include federal/state criminal checks, driving record, and DDS Abuse & Neglect Registry check. 
    • To be checked annually. 
  • The ability to use computer and paper record keeping systems. 
    • Previous experience with Therap (preferred) 
  • Maintain a 2% or lower rate of absenteeism. 
  • Is reliable and arrives to shift on time and leaves shift at the scheduled time. 
    • Must have a personal vehicle to transport themself to and from shift, cannot rely on a rideshare or public transportation (including Uber/Lyft). 
  • The ability to remain awake and alert for the entirety of a scheduled shift. 
Education & Experience Requirements: 
  • A high school diploma or equivalent. 
  • CPR & First Aid Certified (preferred) 
    • Must be able to complete the written exam.
  • PMT Certified (preferred).
  • DDS Medication Administration Certification (preferred).
    •  Must be able to complete the two stage course, exam, and practicum. 
Physical Requirements: 
  • Must be able to bend, stoop, push, pull, reach, sit and walk for periods of time. 
  • Must be able to pass the physical portion of the PMT Certification Course. 
    • Must be able to perform physical management techniques at all times. 
  • Must be able to pass the physical portion of the CPR Course. 
    • Must be able to perform the physical requirements of CPR at all times. 
  • Must be able to lift up to 30lbs as needed.
